In 2014, the Healthcare Cost and Utilization Project found that among Medicare patients, Congestive Heart Failure was the cause of 134,500 readmissions resulting in a cost of $1,747,000,000. This is an average cost of $13,000 per readmission (
Hines et al 2014).
Many studies and meta-analyses have found that self care programs can reduce hospital admissions by 40% or more
(Jovicic 2006).
Thus, assisting patients to manage their own care is an essential component of chronic HF management. This App is based on Evidence Based Guidelines to empower patients to follow a Self Care Program in order to reduce readmissions and hospitalizations.
