This app allows Medicare Advantage Plans to collect HCAHPS and Veterans RAND 12-Item Survey from Members. It can also be customized to include other surveys and data collection types.
HCAHPS
The Hospital Consumer Assessment of Healthcare Providers and Systems(HCAHPS) is a patient satisfaction survey required by Centers for Medicare & Medicaid Services (CMS) for all hospitals in the United States. It is used as part of the calculation for Medicare Stars for MA plans.
Veterans RAND 12-Item
CMS monitors quality of care provided to Medicare Advantage (MA) beneficiaries by through the Medicare Health Outcomes Survey (HOS) by measuring the self-reported quality of life of MA beneficiaries. This is done through the Veterans RAND 12-Item Health Survey (VR-12)
